Gateway to Better Health Provides Care Coordination

The Community Referral Coordinators program is an initiative of the St. Louis Integrated Health Network originally funded by the Gateway to Better Health Demonstration Project to connect uninsured and Medicaid patients in hospitals to primary care homes in the community.

Through this program, Referral Coordinators are stationed in 5 area hospitals, helping navigate patients to primary care or other providers for follow-up and preventive care.  This program focuses efforts on patients with chronic care needs to help improve health outcomes through cost-effective care.

The St. Louis Integrated Health Network also manages a Transitions of Care team, which oversees the Community Referral Coordinator Program.
